APPLICATION FOR AN ACCESS IDENTIFICATION AND PIN NUMBER

for the Office of Professional Development Database Program (OPDDP)

Please complete the information below to obtain your personal ACCESS IDENTIFICATION and PIN NUMBER that will enable you to review your records of professional development sessions attended and reported to the Office of Professional Development. It is also used to enable you to gain online access to your Individual Professional Development Plan (eIPDP).

The information is contained in the Office of Professional Development Database Program (OPDDP) available for downloading on the district’s intranet (network) site. Please be aware that this database is available to you only if you are able to log on to the district intranet (network). A letter detailing your access identification and pin number will be emailed to the address you indicate below.
